SCHOOL REGISTRATION-Medical Registration Information All Children attending a Department of Defense School must have proof of immunity. This requirement is a condition of attendance. A copy of your child's immunization record is needed at the school during time of enrollment. An appointment with the Immunization Clinic can be made by calling: CIVILIAN 0711-680-4316. ® Up-To-Date emergency contact information, duty phone numbers, spouse work phone numbers, e mail address. ® DoDEA Immunization Requirements Form ®Immunization Requirements (See Figure 1 (chart) @ http://www.cdc.gov/mmwr/preview/mmwrhtml/mm5451- Immunizationa1.htm (An External Site) for needed immunizations and schedule. 3 doses of DPT – Diptheria, Pertussis, & Tetanus (at least one after 4th birthday) 3 doses of OPV or IPV- Oral/Injectable Polio Vaccine (at least one after 4th birthday) 2 doses of MMR-Measles, Mumps, Rubella (one at 1 year and the 2nd recommended after 4th birthday) 3 doses of HEPB- Hepatitis B 2 doses of HEPA - Hepatitis A (should be given at least 6 months apart) 2 CHPO-Varicella or a reliable history of student having. 1 dose of Meningococcal Vaccine (MCV4) (given at age 11-12 or at high school entry (15 years of age)) |
